Application for Replacement of Lost or Stolen Learners Permit or Driver Licence Card & PSV Badge
To replace a lost, stolen or damaged valid learners permit card, driving license, PSV badge and Hazardous card, you will need to follow the following process:
Pre–requisites (Prior Condition before Application)
- Valid Learners permit, driving license, PSV badge and Hazardous card
- All overdue Traffic Infringement Notices should be cleared
Checklist (What to bring)
- Completed Application Form.
- Police Report (In cases where valid DL/LP/Badge/DG card is stolen)
- Statutory Declaration (to declare that DL/LP/Badge was destroyed/lost/misplaced/damaged)
- Surrender Valid Driving Licence Card (if your card is damaged or ruined)

Replacement Card Application Process
- Fill in Learners Permit Application Form:
- Obtain and complete the Learners Permit application form online or at the LTA office.
- Submit Application of Issue Form:
- Submit the completed Application of Issue Form over the counter.
- Update Application and Pay Fee:
- Once the application is updated in the system, pay the relevant duplicate fee.
- Issuance of Duplicate Cards (Learners Permit, Driving Licence PSV Badge & DG Card)
- Upon payment of relevant fees, cards will be printed by CSOs and issued instantly.
