Permits can be re-printed and replace provided you produce the documentary evidence. Below is the process of re-printed your permit if it is Lost.

 Pre – requisites (Prior Condition before Application)

  1. Permit should be valid.

Checklists (What to carry with you when at the Counter) [Attach]

  1. Request letter from customer.
  2. Police Report

Important note:

*Police Report to include date of report, signature of the police officer & stamp.
*Correct permit no. to be stated in the police report]

    3. Statutory Declaration by Justice of Peace

Important note:

*
Statutory Declaration to include date of declaration, signatures of permit holder & witnessing officer (JP/Lawyer) & stamp.
*Correct permit no. to be stated in the statutory declaration.

Process

  1. Fill in the application form.
  2. Submit the checklist to the PSV CSO with the fees.
  3. PSV CSO will print the permit.
